What is Hospice Care?

Hospice care is specialized support for terminally ill individuals focused on comfort and quality of life rather than cure. A team of healthcare professionals provides physical, emotional, and spiritual care to patients and their families. The goal is to manage symptoms, maintain dignity, and improve quality of life during the final stages of illness. Hospice care can be provided at home, in a facility, hospital, or nursing home, and is typically covered by insurance. Its aim is to offer compassionate end-of-life care that respects patient's wishes for comfort and peace.

  • Hospice respite care provides specifically to support the families and caregivers up to 5 days to rest and recover of patient living in a private residence.


  • Hospice also covers room and board and transportation to and from the facility as part of Respite Care.
  • General In-Patient or GIP is used to address severe symptoms that need careful monitoring or interventions that can no longer be provided adequately at home.
  • Commonly occurs in hospitals, skilled nursing facility or free-standing facility.
  • This round-the-clock level of care is provided in the home for brief periods of time when the patient is experiencing crisis.
  • Its designed to honor a patient’s wish to remain at home by providing the care needed to control the patient’s symptoms and alleviate the crisis.

At Uni Hospice, we are specialists in end-of-life care. Our hospice teams are trained to understand patients and families as they cope with advanced illness and to manage healthcare transitions to care that aligns with their goals, wishes and values.


How to Refer a Patient to Uni Hospice


Hospice referrals are made by some of the following providers Primary doctor, Nurse Practitioners, Hospitals, Specialist, Discharge planners and Uni Hospice makes the hospice referral process simple.


We make hospice referrals simple and secure. We provide guidelines that help you determine hospice eligibility with confidence.


  1. Determine whether your patient is eligible for hospice care. Hospice has guidelines, both general and disease-specific, to determine patient eligibility. Hospice patients are assessed by their hospice team at regular intervals to determine continued eligibility beyond 6 months.
  2. Have a goals-of-care conversation with your patient and/or their family. An end-of-life conversation with a patient can be difficult, but there are proven ways to make it easier. The best time to talk about end-of-life preferences is early in a patient’s disease. Approach the topic during regular office visits, based on new symptoms, signs of disease progression, options, advance care planning, and facts about how hospice care can help.
  3. Call Uni Hospice at 747-307-3195 to refer a hospice-eligible patient. A Uni hospice admissions professional will answer your call at any time of the day or night, 365 days a year. You’ll be asked to provide your own identification information along with the patient’s first and last name, phone number and city, state and ZIP code.


Patients with advanced illness and their families understand the value of time. Make sure that a timely, well-informed hospice referral gives them time when they need it most.
